League of Legends will be getting its highly-anticipated 4.2 update soon, very likely this week. Today, Riot releases the finalized patch notes, laying out an extensive list of buffs and nerfs to the roster. Top champs including Riven, Thresh, and Yasuo will be receiving slight nerf, while others will see some much-needed buffs.
The entire list is extensive, but included are the two upcoming remakes for Skarner and Xerath. In the case of Xerath, Riot wanted to alter his purpose, making him more of an "arcane sniper" that picks off targets. Previously, players would simply "dump all of his damage" on the nearest target -- basically facerolling. His mana regen and pool have decreased somewhat.

As for Skarner, he was redesigned to become a better jungler, though none of his changes should alter how he's played. Here, Riot was aiming for more cohesion between his skills. For instance, the slow effect from Crystalline Exoskeleton has been moved to Fracture, and his ult will root targets during the charging phase. Overall, he seems to have fewer changes than Xerath.

Aside from the reworks those two have gotten, other champions are getting changes as well, albeit not to the same degree. Worth noting is Riven's nerf/buff. Her health regen has been decreased a bit, from 5.5 per five seconds to 2.5 per five seconds. But her shield has been improved in exchange, from 70/100/130/160/190 to 90/120/150/180/210
Yasuo, the newest kid on the block, will be getting the obligatory nerf all new champs suffer before long. His Flow gain is being decreased, as is the range on his ult.

Also worth noting is Thresh's auto range has been decreased from 475 to 450, because Riot felt his threat level in-lane was just a little too out of control. Be sure to check out the full list of champ changes on the official League of Legends site.
The client itself will also be tweaked, like the complete removal of Journal of Justice.
