This has been occurring for ages, and I did report it a while ago, but the thread's gone missing somewhere.
OK, so you get what I mean, right? Check this picture:

Clearly, the two avatars are different, aren't they? In fact, the first screenshot shows my current avatar, but in the forum window (second screenshot), it's started showing my old avatar. Why is this happening?

The bug may have become more prominent until last night, since we changed where we stored avatars and I only just updated the code last night to flush the avatars on each change.
Strange, when I'm not logged in they're both skulls, but when I am logged in the top one is the hand cursor. But you may be onto something, ACY3. It's strange how this seems to be only happening for me. But thanks guys, I appreciate all your help.
When I quoted your message you were not logged in, and your avatar was clearly the old hand cursor.

Redemption, this may have something to do with the cache overriding feature you use, where you add ?t=123 after the avatar in order for browsers to see it as a new image. Castlewars's avatar URL on the forums contains such a string, but DQ's doesn't.
While viewing DQ's avatar, I just now tried to refresh it several times and it remained as a red skull. However, I then did a force-refresh (Ctrl + F5 in Firefox) and it changed from a red skull to a pointer icon. In other words, from new to old. After that, all regular and force-refreshes displayed the pointer icon.
I then tried adding a question mark after DQ's avatar URL and it changed back to a red skull. This remained the case after force-refreshing. I've no idea what goes on behind the scenes, but the presence of a question mark determines which avatar I see regardless of the cache:
- no question mark (pointer icon for me)
- with question mark (red skull)

yeah, i havent seen this happening to me either, either on firefox, IE, chrome, or on my PSP, and for some reason, i do remember your skull avvy and name when i see it, DQ. has been like that for...what, a few weeks now?
i must mention than this is not happening to me, i dunno or rather i cant recall what was my previous avatar an i havent seen this bug on my side as in my account, i wonder if this is related to people who changed avatars on a specific span of time
You may be right. I have three avatars in my history: a red cartoon skull, uploaded on May 5th, a Brony, uploaded on March 31st, and a Final Fantasy hand cursor which I've had since 2007, and for some reason this is the one it keeps showing, even though I've clearly uploaded two more avatars since then.
This is me speculating redemption but the server migth be having a problem reading the proper dates of avtars or it migth be getting a feed of one date instead of a different one, i will try to check my avatar history but i havent switched all that much avatrs, my first one was of vincent valentine of FF7 followed by one of Big head AKA the mask (the role of jim carrey) and i siwtched to the image of che nearly half a year ago
