This clicks a button and two prongs touch metal contacts on your 3DS and charge the battery.
All the original 3DS's came with them, though obviously smaller than the XL ones. I used my for around a year before the contacts stopped working and I had to switch to plugging in the charger directly. I'm not sure whether mine has a loose spring so the contacts don't touch anymore, or if the plates on my 3DS are too dirty now for a good connection.
Its a convenient feature if you're like me and leave the system in standby all the time and keep it charged, but I wouldn't expect it to hold up forever under heavy use.
