When everyone dies Riki and Rin both can't move forward they instead create their own world where everyone survives. Presumably slipping into a coma..
Welcome to KEY endings, where the happiest one's are slightly convoluted still. the it's not enough. is because you chose the path where riki isn't strong enough to move on afterwards...dun..dun...dun...also this whole thing is crazy debated in Vn communities..

However, wouldn't it make more sense for the Baseball team/game have happened in the real world? That would explain the team being pre-determined as well as Haruka sneaking on the bus, Mio being friendly towards the team, ect ect.
