To get a new game forum created, you have to submit it as a profile, which will automatically create the forum. Read here for more details.

If there is already a profile for it, but on a different platform, make sure the nickname (after you submit it, you can go to the queue and edit it to change it if necessary) is the same as the existing profile. To see the nickname just go to the main profile and look at the last part of the URL.
