If Drive-thru's actually did anything, or food in general did, then perhaps I wouldn't mind having this added in.

In Saint's Row, I recall being able to hold 4 items, and one of them was food which would be able to heal you. Personally, I found this made the game too easy, but if GTA IV implements something similar, then I suppose having drive-thru's wouldn't be a bad idea.

But I do like the suggestion of having NPC's still travel through them, even if it's just for random dialogue and such

You brought up another great idea in your post. Things like Coffee could have a similar effect to Adrenaline Pills did in previoous GTA Games.

If this game offered something similar to that, it'd be great. I miss Adrenaline pills

YOU as LUIS pull up say YEA I WANT A DUBBEL SHOT BUGER a SO AND SO SORT DRINK and a ORDER OF FRIES/ NEXT it is H after all he normaly was beside you in the car HE leans over SO he can be heard and says YO I WANT A BLEEDER BURGER A HART ATACK SET OF FRIES and a DROWNER DRINK a BURGER SHOT TAKE ON A 64 OUNCE DRINK NOW it's A's turn HE orders a BIT OF WHAT L got and A got.

NOWW think of this as you go on this man date U can order 4 all the folks in the car LIKE SMOKE DID in GTA SA the FUNNY LINES would be GREAT TO HERE AGAIN PLUS the houmor of YOU trying to keet all the food right and who got what would make 4 some FUNNY SAYING and reaction IF this could happen .

IF it was a place here then 4X4s quads and suvs and other county stuff should pop up LIKE BIG RIGS DELEVERY TRUCKS like a YANKEE and so on and so forth
