I had maybe two hearts left. I was walking around in Faron Woods when I noticed that Coro has something scoopable in his pot. I got some and, not noticing the resemblence to the chu jelly of death, drank it in hope it would help heal. Coro watched as I died.
Should've just went to the spring....

Mine is kinda complicated. I tried to throw a bomb at a bat (stupid move on my part), bomb misses, it roles down the ledge behind the bat, explodes in my face, and sends me backwards over an edge into the lava, and to my death. I'm not joking.

I was happily riding the horse in hyrule field at night. You know that poe on the top off like a cliff thing. Yeah well I saw it, jumped off my horse, transformed and hit it once. I wasn't locked on so next thing I know I'm speaking to the horse! I'm thinking, "why was a poe just talkin' to me?" not noticing it's destroying me.

I've got another one my friend achieved:
He had finished getting the eldin bridge back from the desert, and his sister tripped over one of the exposed wii wires which unplugged it and turned it off. He went back on to find himself still in the desert, but forgetting he hadn't done the bridge due to it turning off, he warped to Eldin bridge and fell far to his doom.

One more I decided to bring upon myself earlier.
I suddenly remembered diababa's room. When your beating him the waters purple and hurts you if you touch it, but I'd suddenly remembered it turning blue. So I went to the boss room earlier, zora armour eqiupped, and took a dip in the VERY blue water. I went under to search for chests, and saw blank whiteness. Surely beiliving Diababa couldn't have come from nothingness, I began to swim down. I died.

I think he may have stolen the cake...
