I definitely got the vibe that he hadn't been to the bar for a looooong time. And his Geostigma seemed to be flaring up pretty badly during the movie, so he'd obviously been avoiding people (especially Tifa) for a while, so as not to worry them about the "Geostigma attacks" that he kept on having. (Tifa didn't know he was sick. 	Tifa would give Rinoa the beatdown... as fas as looks.. meh... maybe rinoa.. but that has a lot to do with the fact that she's at least anatomically correct
