The proper way to get the Maniac Mode (in the Practice menu) is to complete Vega's/M. Bison's combo set.
The other way is the cheat it:
From the Main Menu , highlight Practice selection and press Select (x5), Left, Select (x5), Down, Select (x3), Right, Select, Down, Select (x4).
From the Main Menu , highlight the Practice selection and press Select, Down, Select, Left, Select, Up, Select, Up, Select, Right, Select, Up, Select, Left, Select, Up, Select, Right, Select to unlock samples in maniac mode.

From the Main Menu, highlight the Options, then hold Select and press Start.
Unlocking most of the special characters involve completing a combo set. Usually a message will appear that states a bonus character has been enabled.

From the Main Menu, highlight the Bonus selection and press Select (x5), Right, Select (x3), Left, Select, Right, Select (x2).
From the Main Menu, highlight the Bonus selection and press Select (x13), Up, Select (x4), Down, Select (x13). (yeah, it seems excessive to us, too :P)
Complete Hayate's combo set. You should see a message, which means you the "Extra Bonus Game Mode" option has been unlocked in Arcade mode, and the Excel and Satellite bonus games.
