When you finish a mission from Jones, or Smith, you can grab a item with your mouse and carry it over to your 'Mission Log' and hit Back to city with it. In the chart which tells you your exp, money, ect.. An item will be there, in your mursor hairs.. After your done with that hit OK and there you go, an item created from another item.

Grab a med-pack hold in your mouse, than go to mission log, click back to city, and Bam. =) Easy as that.
