Level Select: Press Up, Down, Left, Right repeatedly at the title screen, followed by one of the following:

At the Title screen press Up, A, Down, A, Left, A, Right, A. Begin the game, press Start to pause, hold B and unpause to resume gameplay in slow motion.
Press A (4 times), Up, Down, Left, Right when "Press Start Button" appears. A chime will sound so you know you've done it right. Then hold B and press Start and another chime will sound. Hold C and press Start one more time and you will begin the game and be invincibly to most things. Falling down bottomless pits, being stood on by the end boss and the green hands from Level 4 can still kill you, other than that you're invinicble.
